Western Kenya has a high morbidity and mortality rate. It is a malaria and HIV/AIDS endemic area. Other common conditions include TB, diarrhea and respiratory infections. Drinking water and sanitary facilities are problematic, causing frequent outbreaks of cholera. More than 40% of pregnant women give birth at home with the help of untrained traditional birth attendants, often under poor hygienic conditions.

For all these reasons, SWAP is always working to help people in the region stay healthy and move forward. For example, it is not only about providing clean drinking water, but also and especially about developing locally produced water purification methods, which are distributed through local groups to everyone who needs them. SWAP works together with other agencies and strengthens the social infrastructure. ​
Field workers
SWAP outreach workers identify a 'problem case' and invite him/her to share his/her story via a video. The outreach workers, make a short description and film recording of this. This is then submitted as a project to the global crowdfunding platform DonorSee and publicised via the SWAPkenya.nl foundation. As soon as the required amount is received, it is distributed to the recipient and an explanation is given of how the money was spent with another short film recording.
